Abstract:

Background: Diabetic foot is a serious complication of diabetes mellitus, which may lead to foot ulceration and amputation. Poor self-management is a major risk factor for adverse outcomes. The integrated Promoting Action on Research Implementation in Health Services (i-PARIHS) framework supports the translation of evidence into clinical practice. This study aimed to construct an evidence-based self-management program for patients with diabetic foot and test its preliminary application.

Methods: A mixed-methods approach was used. qualitative interviews were conducted with patients, nurses and experts to explore barriers and facilitators of self-management. A systematic review was performed to synthesize relevant evidence. Guided by the i-PARIHS framework, we developed the tailored intervention program. Baseline data of participants have been completely collected.

Results: The program includes foot care, medication guidance, blood glucose monitoring, diet instruction and psychological support. It adapts evidence to local clinical context and patient’s actual needs. Baseline characteristics of enrolled participants were obtained for further intervention.

Conclusion: The i-PARIHS-based self-management program is feasible. It has potential to improve diabetic foot patient’s self-management capacity and reduce foot complications. Further implementation and outcome evaluation will be carried out.

Wang Jie is a postgraduate student majoring in nursing at the school of nursing, Zhengzhou University. Her research interests include evidence-based nursing and chronic disease care. Her qualitative study explores the barriers to self-management among patients with diabetic foot from the perspectives of patients, caregivers, and healthcare providers.
