Abstract:

Background: Teamwork is vital for nurses facilitating patient transfer. Improving the perception of teamwork through TeamSTEPPS was a necessary and incredible feat

Problem: Transfer center nurses had worked from home for a year and a half before they had a change in leadership, who implemented a transfer center, requiring nurses who were working in silos to co-locate in person, in a new department. Initially, to improve the perception of teamwork, the goal was to update the onboarding binder to improve this process for Experienced Registered Nurses (ERN), using TeamSTEPPS mutual support. During the planning phase, it was discovered that a transfer nurse educator had completed that process, requiring pivoting to another intervention; creating an SBAR Intake Form for the transfer center nurses.

Methods: Reviewed the intake process and the eight intake forms used by the transfer center nurses. assessed the intake information that was obtained on these forms and discovered there were no standards and guidelines for the intake information. 

Intervention: Created a standardized evidence-based TeamSTEPPS SBAR Intake form with four revisions based on the nurses' feedback. The final version was decided on by week 6 fo the implementation

Results: A final version of the evidence-based SBAR Intake Form was created. Teamwork perception was measured using the pre- and post-surveys Healthcare Team Vitality Instrument (HTVI) questionnaire and analyzed by a statistician

Conclusions: The perception of teamwork is essential to improving communication and patient safety during transfers into a healthcare system. Having a standardized SBAR Intake form improves the perception of teamwork through improving communication
