Title : Development of an education visit process for the safety of homecare clients
Abstract:
Purpose: With the advancement of healthcare, there is a growing trend of patients being discharged to their homes with various medical devices and tubes in place. This has resulted in an increased psychological and emotional burden on caregivers due to unfamiliarity with these medical devices and the challenges associated with tube management. It is essential to classify patient groups that require precise knowledge and efficient management methods, providing convenient and standardized education at the appropriate time. The development and implementation of an educational visit process are anticipated to enhance understanding of new medical devices and tubes, easing caregiver burdens and promoting stable adaptation at home.
Methods: Six key topics requiring educational visits have been identified: T-can, PG, L-tube, ventilators, tube management (PTBD/PCD/PCN), and end-of-life patient care. Educational materials have been developed for these topics, and simulation kits have been crafted for post-discharge home visits. The goal is to conduct necessary education, evaluate nursing performance, and measure satisfaction levels.
Results: As the severity of patients at home increases, and with the apprehension associated with discharge due to insufficient caregiver education, this activity was initiated to prioritize the safety of patients at home. As a result, we have developed six education visit processes, and completed the production of educational materials and equipment kits successfully. In addition, the knowledge and performance scores of patients and caregivers before and after the education visits showed a remarkable increase, rising significantly from an average of 45 points to 98 points. Post-implementation satisfaction scores were also highly successful, reaching close to a perfect 99 out of 100 points.
Conclusion: Through the systematic implementation of these processes, patients can be discharged from the hospital without undue stress, and patients and caregivers at home can adapt well to the new environment, leading to a safer lifestyle. The outcomes of these educational visit programs are expected to contribute comprehensively to the enhancement of healthcare services and the practical implementation of more efficient and secure home care.
