Title : The implementation of a prediabetes risk test in a primary care setting

Abstract:

Type 2 diabetes is a significant public health concern, with prediabetes serving as a precursor. Early detection and intervention are crucial in preventing its progression. This Doctor of Nursing Practice (DNP) project aimed to implement the Centers for Disease Control and Prevention (CDC) prediabetes risk test in a primary care setting to enhance early detection and facilitate timely interventions for at-risk individuals. The project aimed to improve patient outcomes through early identification of prediabetes and subsequent referrals to registered dietitians for tailored interventions. The project followed the Plan-Do-Study-Act (PDSA) Cycle and involved a multidisciplinary team, including nurse practitioners, medical assistants, and registered dietitians. Staff education sessions were conducted, and the risk test was systematically integrated into routine patient assessments. Data on screening rates, referral success rates, and patient outcomes were collected and analyzed. Implementing the CDC prediabetes risk test in primary care is an effective strategy for enhancing early detection and intervention for individuals at risk of developing type 2 diabetes. This project highlights the importance of preventive strategies and interdisciplinary collaboration in improving patient outcomes and reducing the burden of type 2 diabetes on healthcare systems.
